Cardiocom (Minneapolis, Minnesota) reported that Ucare Minnesota, a non-profit health maintenance organization, renewed its multi-year agreement with the disease management company to provide systems for remote daily home monitoring of Ucare’s congestive heart failure (CHF) patients.
