Milford Criteria for Cardiopulmonary Rehab
Milford (DE) Memorial Hospital Home Health Care has established the following criteria for congestive heart failure patients to be referred to cardiopulmonary rehabilitation:
The goal of the home health services to the patient with congestive heart failure is to maintain stability and increase endurance to enable the patient to enter the cardiopulmonary rehabilitation program as an outpatient in two to three weeks.
The following criteria must be met for three consecutive visits:
1. Stable vital signs:
Resting systolic blood pressure less than 200 mm Hg.
Resting diastolic blood pressure less than 120 mm Hg.
Oral temperature less than 99 degrees F.
Respiratory rate 16-24.
2. Oximeter reading equal to or greater than 90%.
(Taking into consideration those patients with chronic obstructive pulminary disease component, saturation may be stabilized by supplemental oxygen at 2 liters via nasal canula.)
3. Dependent edema less than 2+.
4. Demonstrated ability to ambulate 100 feet.
5. Weight gain equal to or less than 3 lbs. in 1 week.
6. Digoxin, potassium, magnesium levels within normal limits.
7. No PRN diuretic injection/oral use.
8. No evidence of infectious disease, i.e. TB (active), mononucleosis, hepatitis, AIDS.
