A 52-year-old male presented to his primary care physician in February 2006, complaining of decreasing exercise endurance. The patient was a runner, and typically ran 5-8 miles three or four times a week. During the past month, he noted difficulty in completing the longer runs, and initially attributed this to a persistent cold and URI.
