The idea of patient safety is not a novel concept, but in the years since the publication of the 1999 report by the Institute of Medicine (IOM; Washington) that laid the blame for 44,000 to 98,000 hospital deaths each year on deficient medical care, patient safety has taken on an urgency perhaps not seen since Hippocrates issued the imperative "First, do no harm."
