Transitional care tools available

The final package of best practice intervention tools for transitional care coordination is now available from the Home Health Quality Improvement Organization Support Center. Best practice intervention packages are free and include educational tools and resources, guidelines, success stories, and best practice education to assist agencies in reducing avoidable hospitalizations.

The campaign resources are available on the Home Health Quality Improvement web site at www.homehealthquality.org. In addition, campaign resources are available on Medicare Quality Improvement Community web site at www.medqic.org.
