CHF patient follow-up via automated phone calls
Here’s a way to stay in touch, by phone, with congestive heart failure (CHF) patients. Heart Success, designed by Greenwood, IN-based TeleHealth is a multidisciplinary program that asks patients simple yes/no or number-oriented questions. They complete the two- to four-minute phone interviews from their homes. Areas covered include shortness of breath and fatigue as well as other key CHF symptoms. At the conclusion of a call, the patient may page a nurse and expect a call back within two to four minutes. Or the program automatically pages for help if patient responses indicate problems.
If the signals are urgent, the call center can immediately dispatch a provider to the patient’s home.
Other home-monitoring programs offered by the firm include anticoagulation management, high-risk obstetrics, and chronic obstructive pulmonary disease.
